UPS Chartered Plane Crashes – 3 Killed

The Philippines is the site of a UPS chartered cargo plane crash on Wednesday which killed three employees. The plane carried three crew members and three off-duty airline employees.

The public relations manager for Louisville-base UPS Airlines announced the Russian-made Antonov-12 aircraft was owned by Interisland Airlines. The plane was flying an inter-Philippines route when a fire broke out and the plane crashed before it could make an emergency landing.

UPS Airlines is a division of Atlanta-based UPS and charters planes around the world where it does not hold operating rights or where its aircraft are too large to service certain airports.

NTSB Releases Preliminary Report On Medical Helicopter Crash

The pilot of a Eurocopter AS-350-B3 operated by Memphis Medical Center Air Ambulance Service (dba Hospital Wing) was “trying to beat the storm” when the helicopter crashed outside Brownsville, Texas on March 25, according to the NTSB.

The helicopter was returning to the Haywood County EMS Heliport in Brownsville after transporting a patient in Tennessee when it crashed, killing the pilot and two flight nurses.

According to a shift pilot at the home base heliport, he and the Hospital Wing pilot had discussed a front coming from Memphis towards Brownsville and the helicopter pilot stated he had about 18 minutes to get the chopper back to base to “beat the storm.”

In the last communication between the shift pilot and one of the flight nurses, she told him they were “30 seconds out.”The wind had then picked up to 20 knots and after he hung up the phone, there was an “immediate” loud clap of thunder and lightning – he looked out, but saw no helicopter.

Car Crash Wrongful Death Suit Settled For $1.75M

The family of a 14-year -old girl killed in a car crash more than five years ago has settled their wrongful death lawsuit against the family with whom she was visiting for $1.75 million reports the Richmond Times-Dispatch.

When the 14-year-old’s father dropped is daughter at her friend’s house, he specifically told her friend’s mother she was not to ride in an automobile with a juvenile. He told her “no boys with cars” and was told “not to worry.” Later, the mother told the girls to accept a ride home from the movie theater with a young boy, despite knowing he had a reputation for recklessness. During the ride home, the young girl sent her family a text message she feared for her life.

The Virginia Supreme Court stated that although supervising adults can’t be expected to ensure a child’s welfare, they are liable when they fail to act responsibily.

$2.255M To Be Paid By Officer And Bar In Deadly Car Crash

A police officer and the bar which served her will pay a total of $2.255 million for the wrongful death of four and the injuries to another man on March 21, reports St. Louis Today.

The lawsuit claimed the police officer had consumed a “high quantity” of alcohol at O’Leary’s Restaurant and Bar and the employees at the bar knew she was intoxicated and did not stop her from driving or call her a cab. Despite her slurred speech and unsteady walk, the bar continued to serve her alcohol. After leaving the bar, the officer crashed into a Honda with five students from India.

The driver of the car hit by the police officer suffered head injury, fractured rib, liver trauma, lung contusion, and contusions to the skull. His four passengers were killed.

Plane Crash Which Killed DJ AM Caused By Under Inflated Tires

The National Transportation Board has concluded the 2008 plane crash which killed four people and left Travis Barker and the late DJ AM injured was caused by under inflated tires.

All four tires exploded as the plane raced down the runway for takeoff. The pilot (with only 35 hours in this plane) then tried to abort takeoff, but due to sensors in the plane, that was impossible and the plane burst into flames after hitting an embankment.

Tires should be changed after eight days if proper maintenance has not been carried out. This Learjet’s tires had not been checked for three weeks prior to the tragic flight. The investigation found operators of charter flights aren’t aware of how quickly tires can lose pressure and that the FAA and Learjet Inc. didn’t take aggressive enough action to fix a design flaw on this model of jet following a similar accident in 2001.

Proper maintenance is crucial to flight safety – no item is too small when dealing with people’s lives. NTSB Chairwoman, Deborah Hersman, stated, “This accident didn’t need to happen.”

The families of Travis Barker and Charles Still have settled with several companies over the accident and AM’s estate settled a $20 million lawsuit in January.

$3.8M Paid To Man Injured In Boating Accident

The Dallas Morning News reports a federal jury has ordered a boat manufacturer to pay $3.8 million to a man who was injured in a boating accident on Lake Austin.

An 18-year-old lost part of his leg while he was wakeboarding on July 4, 2007. The lawsuit alleged the boat, made by the Brunswick Corp., did not have proper safety devices in place to prevent such an accident. The suit claimed the young man’s leg became tangled in the propeller because the boat did not have guards and covers to protect him from becoming stuck in the propeller.

The company is considering an appeal.

NTSB Begins Investigation of Fatal Plane Crash

The investigation begins into the April 1 fatal plane crash at Dayton-Wright Brothers Airport that killed a 50 year-old father and his 21-year-old daughter.

According to the NTSB, a preliminary report will be released in about 10 days, but complete report will take a year or more.

The Chicago Sun-Times reported on April 2, the daughter was a student at the University of Dayton and a member of their women’s volleyball team. The Beechcraft Bonanza crashed as it tried to return to the runway after the pilot reported a cargo door was open.

Oregon Bar Owners Will Pay At Least $1.3M In Drunk-Driving Death

The former owners of a restaurant in Gresham, Oregon will pay $1.3 million after being accused of serving too much alcohol to a convicted drunk driver who killed two young women reports KATU.com. The judge could more than double that amount.

Although the Golden Star Restaurant is now under different ownership, the parents of the young women sued the former owners accusing them of knowingly over-serving the driver and then allowing her to drive.

The bar had a history of over-serving its patrons and had received several written warnings from the Gresham police, an Oregon Liquor Control Commission fine, and dozens of patron DUI arrests.

The two young women were celebrating the 21st birthday of one of the girls when their car was hit head on by the drunk driver going the wrong way on the highway. Neither girl had alcohol in their system.

General Motors Recalls Vehicles Due To Fire Danger

General Motors is recalling the 2010 Chevrolet Express GMC Savana built in
February and March because the alternator could cause a fire, reports U.S News and World Report.

GM is stopping the production and sale of these two vehicles until a solution can be found and the problem fixed. The recall affects about 5,000 vehicles of the 2500 and 3500 series heavy-duty models. Among all the recalls in the auto industry over recent months, this is one of only a few in which the auto maker stopped all sales of the product.

Three fires so far have been related to the alternator, with been no injuries or accidents reported. Owners of these models are asked to stop driving them, park them away fro buildings and other vehicles and to disconnect both battery cables.

$33M Award Against Walgreens Upheld In Woman’s Death

The Ledger of Lakeland, Florida reports an appeals court has upheld a lower court’s award of $33.3 million to a husband and three of his children in their wrongful death lawsuit against Walgreens. Their 46-year-old wife and mother died in 2007 after Walgreen’s pharmacy mistakenly gave her 10 times the prescribed dosage for a blood thinner to treat breast cancer.

Diagnosed in 2002 with breast cancer, she was given an 88 percent chance to recover with the proper treatment. Part of the treatment was a prescription for Warfarin. At Walgreens, an inexperienced pharmacy technician gave the woman 10 milligram tablets instead of the prescribed 1 milligram tablets.

After several weeks of taking the medication, the over dosage led to a brain hemorrhage and her being paralyzed, capable of communicating only by blinking her eyes. After a year of hospitalization, the woman was too weak to continue treatment for cancer. Her cancer spread and she died in 2007.
